42 yrs detected + in Oct `06 with CD4=58 and VL=0 ! After my other question you have said that is is a little weird to have a CD4=58 and a VL=0 WITHOUT THERAPY.
WHAT DID YOU MEAN BY THAT ?
P.S.: I have started therapy on the 10th Oct.
Ziagen , Epivir , Stocrin , Bactrim and after a 1,5 month my CD4=58 .

It is quite unusual, but by no means unheard of, to have a very low CD4 count and undetectable viral load (without treatment). Most patients who have HIV disease progression will have viral loads in the thousands if not higher; it's also generally unusual for those with undectable viral loads to have significant disease progression. Perhaps I misunderstood your original post, whereby you may have had a detectable viral load prior to treatment (the usual situation).
I'm happy to hear that you started on treatment; I hope that the abacavir (Ziagen), 3TC (Epivir) and efavirenz (Stocrin, Sustiva) are treating you well. Since it has been 6 months since you started, you're already out of the window (usually within 3 months) of having immune reconstitution syndromes happen. The best way to avoid reconstitution syndrome (IRIS) is to make sure that as best possible, that all active infections are diagnosed and on treatment before antiretroviral medications are started-- again, for you, a moot point.
