Great question. Taking Truvada every day will greatly reduce the risk of getting infected with HIV. Whether or not you can get away with less than daily dosing of the pill is a subject of much discussion and some research.
While daily dosing is accepted as being the most effective, it looks like taking Truvada at least 4 days out of 7 works, at least, almost as well. A recent study from France looked at using Truvada right before and after sex. Men took 2 tablets of Truvada 2-24 hours before sex, then 1 tablet 24 hours later and then 48 hours after the first dose of the drug. This reduced the risk of infection by almost 90% compared to a control group - even though a lot of men did not follow the instructions faithfully.
So, I recommend the following:
- Make sure you are really HIV negative now and do not harbor other STIs like gonorhea, chlamydia, syphilis.
- If you are HIV neg and plan to have unprotected sex with people who could be HIV pos, consider Truvada PrEP.
- Take Truvada daily if possible. If not, then get in at least 4-5 pills a week. The French method above is also an option if you can keep up with that schedule.
- Be aware that PrEP does not protect you from other STI so make sure you get checked for those regularly.
