Should I consider switching from the tenofovir to something else? I'm on omeprazole so my options here in Bangkok may be limited.im id after 6 months and cd4 400

Answer

You don't indicate what your serum creatinine and BUN were prior to starting Atripla, and whether you have any other underlying medical conditions that could be affecting kidney function. So it is important to know where you were starting from and if this is a real change or not. There are two forms of tenofovir available: Tenofovir disoproxil (which is the form in Atripla) and tenofovir alafenamide, which is now in several new preparations and causes less kidney issues than the other kind of tenofovir. This may or may not be available where you live. ‎Assuming this is a real change, yes, you could consider changing medications.
