Hello. I have developed some kind of Gastro condition, possibly due to the taking of Antibiotics for 2 months last year to combat a Acne Breakout. I have tried Antacids, Rinitidine and Nexium. So far only Nexium have helped tremendously. However I had to off of it because my Pharmacist discovered taking Nexium with Complera is contraindicated. I wish my doctor would have told me this from the beginning now he's suggesting I switch from Complera to Stribild. My concern is while I do want to get rid of my stomach pains badly (it's been happening for over 2-3 months now), having to change from Complera to Stribild seems drastic just because I need a Acid Reducer that works. My Gastroenterologist seem to think that I can take Complera in the morning and Nexium at night. My Primary Care Physician says I shouldn't do that. What do you think? I'm reluctant to change my HIV regimen because I just changed from Trizivir to Complera last April. Thanks in advance.
The recommendation based on reasonable data is to avoid potent acid reducing drugs such as Nexium when taking Complera since the levels of the rilpivirine may be reduced enough to increase risk for virologic failure. Though some patients may do OK separating Nexium from Complera dosing there is no dependeable way to predict who will do OK (drug levels not available) so the prudent move would be to switch to an ART combination that can be taken with Nexium if it is believed that you would benefit from its use. Most patients would do OK with a switch from Complera to Stribild (or Genvoya) as well as other switch possibilities (such as to triumeq if HLA B5701 negative). KH
