My father is ckd htn and hiv positive cd 4 is 55 ad viral load is 5800 please guide me

Your father's CD4 count is indicative of very advanced HIV disease and AIDS. A viral load of 5800 is relatively low, but certainly enough to cause further worsening of his health.
For purposes of this discussion, I'll assume that he's not yet on HIV treatment- and if so, he should start on HIV medications (antiretroviral therapy, or ART) as soon as possible. With ART, his viral load will become suppressed and his immune system will have the best chance at recovery. Today's ART is usually very well tolerated and dosed (for most people), once-daily.
If by CKD you mean that he has chronic kidney disease, this will need to be taken into account in the selection of his ART as some medications require special dosing for people with severe CKD, and others (like tenofovir) might need to be avoided- but this depends on the severity of his CKD.
At this point, the best thing you can do as family is to support his getting to his doctor's appointments and encouraging the initiation, and adherence to ART.
