Alcohol, drugs (including prescription medications), or poisons can also cause hepatitis. That's a very low viral load is the cirrhosis is due to HBV infection. Given that the antigen test is positive though, HBV is the likely cause unless there are other causes (such other viruses, alcohol, other medical diseases, etc)...
