TODAY I RECIEVE MY NOTICE FOR MY NOSE SPRAY FROM THE PATIENT ASSISTANCE PROGRAM AND THEN AT THE DAP PROGRAM THAT THERE WILL BE A $5 PROCESSING FEE FOR EACH MEDICATION THAT IS PROCESS THRU THE DAP PROGRAM,SO WOULD SOMEONE TELL ME WHAT`S GOING ON NOW...

So I have been positive since July 2012, have been on meds since December 2012 thanks to ADAP, I go to my appointment tomorrow for eligibility. Since I was already eligible once, is there any paperwork I need to bring besides my recent paystubs?
