Rashes due to nevirapine are fairly common (seen in around 20% of patients) and can be very serious with 5-10% of patients stopping the drug. Rarely can be life threatening so careful monitoring for liver toxicity and involvement of mucous membranes... Read more »

If your liver enzymes are elevated due to nevirapine then a switch to Atripla (substituting efavirenz for nevirapine) often would be beneficial. There are a range of issues that result in elevated GGT levels. GGT is sensitive to biliary tract... Read more »

Efavirenz often will increase plasma lipids more than nevirapine. I often would switch to atazanavir (+/- ritonavir). Use of ritonavir would hinge on whether there is any history of resistance or need for stomach acid drugs. I also might switch off... Read more »
