Esteatosis Hepatica

Question:

Doctor, estoy tomando stocrin(sustiva), DDI, y Zerit(efvirenz), pero me diagnosticaron esteatosis hepatica; en este momento suspendi la terapia. Podria indicarme si este diagnostico perjudicara mis futuros tratamientos o si puedo continuar con la terapia que actualmente estaba tomando? LLevo 5 dias con el malestar (dolor en el higado, nauseas, dolor abdominal y gases en el estomago), es esto normal? Un medico amigo me indico que lo mas conveniente sea practicar la prueba de hepatitis C. Doctor Usted que opina?

Definitivamente hizo lo correcto. La Esteatosis Hepática en muchas ocasiones esta asociada con la acidósis láctica las cuales se han visto mayormente asociadas con el uso combinado de Zerit (d4T) y Videx (ddi). Le recomendaría que una vez los síntomas resuelvan discuta con su médico las opciones futuras de tratamiento. Es recomendable como estrategia futura utilizarlas por separado con AZT, 3TC o Abacavir (Ziagen). No obstante esto no significa que no pueda volver a utilizarlas para futuras opciones de tratamiento y que esta condición se vuelva a repetir.

En cuanto a descartar Hepatitis definitivamente debe ser parte del diagnóstico para excluir Hepatitis A, B, C como también medicamentosa, ya que se sabe que el estocrin o sustiva puede en algunas ocasiones causar hepatitis.
