lesiones en piel tipo micoticas? - JS

tengo hiv Carga Viral CD4 500 cel/ml y carga viral 138000,me han salido lesiones en piel tipo micosis ;Mi terapia es Comvivir y abacavir(ziagen); fluconazol 150 mgs semanal y sulfas interdiario (lunes,miercoles y viernes)800mgs me hice un cultivo de hongos y bacterias en piel y salio negativo a hongos(suspendiendo el fluconazol y las sulfas por una semana)y positivo a Corinebacterium sp, me estoy tratando con gentamicina crema y ciprofloxacina 500mgs por 8 dias las lesiones han mejorado pero no del todo quisiera su opinion al respecto gracias

Me resulta difícil precisar qué tipo de lesiones pudiesen ser. Si sus CD4 estan alrededor de 500 encuentro muy poco probable que sea relacionadas a una enfermedad oportunísta relacionada al SIDA. Por otro lado si su carga viral es 138,000 me sospecho que su tratamiento antiretroviral no está siendo muy efectivo y debe de considerar un cambio. Le recomiendo una biopsia en forma de cuña para esas lesiones, ya que pudiesen resultar ser de otra etiología. Medicamentos que usualmente se utilizan para Corynebacterium lo son: Vancomicina, Rifampicina, Linezolid, entre otros.
