Protects sex with no Condom tear 100 % safe??

I read that condom provided 100% protection if it was used correctly and there was no slippage and breakage. I had a huge relief as I had protected sex with a possibly infected csw.
But trying to understand more about HIV, I went into a lot of forums, and in some on Tested positive and living with HIV, I saw a few cases of people getting infected even after practicing safe sex and using Condoms and making sure their condoms never broke, They have posted they have no clue on how this might have Happened.
So what does it mean by Condoms are 100% safe when used correctly and not broken?
