Protected Sex with an Escort
      #12121 - 11/10/00 09:00 PM

A little over a month ago I had protected vaginal sex with a girl I later found out to be a "high class" escort. She travels frequently to be with her clients.

In any event we had vaginal protected sex (female superior)in which she used water based lubricant since she was "dry". She put on the condom. The entire event lasted about 5 minutes. The condom did not break or slip.

Didn't think anything of it until I got what seemed to be a cold about a week ago. No fever, swollen glands, or rash. Just scratchy throat (not sore), minor bodyache, mostly shoulder and up - for a day. Then coughing and some minor post nasal with a bit of a raspy throat. Total of about 3 1/2 days.

I contacted her and asked her about her HIV status. She stated that she always practiced safe sex and that she got regularly tested. She stated that she was HIV-.

Questions are these:

1. How do you rate the exposure based on my explanation? Should I get tested?

2. I noticed that even when the condom is on completely, there still is some of the penis and or course the scrotum that is unprotected. The penis is very vascular and there are alot of veins near the surface of the skin. What is the chance of entry there? I didn't have any cuts or abrasions that I know of. Does the skin in that area provide adequate protection? I've got a large vein that enters the penis on the top from my trunk.

Your thoughts would be appreciated. I've kinda freaked every now and then about it. It's been 49 days.

Sometimes I think the mind can mimic symptoms and convince you that you've got something. As indicated by several on this forum. I've had alot of anxiety and weird "pains" that I want to translate in ARS, and AIDS.

Any thoughts would be appreciated.

God bless you all.

Friend,

I think you are FINE. I have also posted similar questions and I have also been in the company of escorts in the past 7 years or so of my life. Many men do it for many reasons, but that is another topic.

SO-

1-You used a condom and it did not fail..This is GREAT. The virus needs to to have access to the bloodstream. Do not worry about the virus getting thorugh some unprotected skin, the virus, as long as the skin is unbroken and their are no open sores, cannot penetrate skin.

2-The chances of getting infected using a condom, and it did not fail are so unlikely, that I don't think it has ever even been measured. There are studies that speak to discordant couples ( one +, the other -), and with 100% consistent use of the condom none became infected. Many escorts, insist onusing condoms, and there have been studies thst find that escorts ( sex workers) do not have a higher than normal level of HIV infection. Ryan Kull, who runs a message board on this system speaks to that. I suspect that street walkers who are drug addicted are the riskiest group. I am attaching his thoughs on that.

3-I also worry all the time about becoming infected, but am to damn scared to take a test..thats my curse in life.

4-I KNOW symptoms in many cases are caused by stress, and right now there has been a cold going around. Try to not relate symptoms to HIV infection. It is NOT a reliable way to decide anything.

5-There have been other posts that I think you should read, some by me an some by others in the last 60 days on this same topic..go and read them!

Hope this helps...If you would like to talk more..post again..

Hi Ryan,

First and foremost, keep up the great work - the level of detail that your go into in your responses is greatly appreciated!! I have a tremendous amount of anxiety about the following issue/question: 1) In response to "using a condom..but" (dated 9/20/00) you state that there are many myths/misconceptions about sex with prostitutes -- can you please go into a little more detail regarding these myths and misconceptions?

Ryan Kull's Response:

The word "prostitute" often conjures up negative, stereotypical images that cannot be applied to a whole group of people (whom I'll call sex workers from now on). Sex workers vary in gender, socioeconomic background, race, education, sexual orientation, as do their individual behaviors and personalities. There are many sex workers with whom I've talked who actually use condoms very consistently for most, if not all, sexual activity (I'm referring to oral sex). Additionally, sex workers in certain studies had lower rates of condom failure than the general population. While there is an increased risk of sex workers coming into contact with STDs, this information would suggest that many sex workers can and do protect themselves.
From working on this forum, I've observed an interesting pattern. People frequently report low-risk sex with "prostitutes" and express inappropriately high levels of anxiety. I think the disease anxiety and guilt people experience after a safe encounter with a sex worker has more to do with unconsciously associating sex workers to disease than with the actual risks involved. The mind can really deceive us into thinking that we are at high risk when we are really not, and vice versa.
A condom used correctly with a sex worker provides the same protection that it would when used with a non-sex worker. The person's trade does not reduce the condom's effectiveness.

Thanks for the comforting words. They're appreciated.

I guess in thinking about it....why would she insist on using a condom at all if she were HIV +???

Secondly, I can't believe that she produced much if not any vaginal secretions. She used water based lubricant and obviously was not "excited".

What do you think of the posts of Captain Japan and his inferences to the transmission female to male?

With regard to seroconversion...I can't believe that a disease like HIV would not produce significant lymph swelling with fever during the process for a decent amount of time. Not just a day or so. I've had neither. Not a single swollen node or fever but from what I've read I guess this could happen. What do you think?

Let me know your thoughts and some of the issues you've dealt with.

I am concerned about the 1/2" or so of my penis that was exposed - the fact that it is very vascular and those veins are closer to the top of the skin.

Thanks and God Bless.

Do not worry about that section of your penis. If there was NO open sore, you are fine. the skin is a very protective barrier, and even the CDC which is VERY conservative says this!

I have dealt with the same issues around extreme anxiety, becuase she was an "escort", which in mnay cases are the same women you might meet in a bar or in a park etc.. and take home..is the protection and sex any different?..nope...just in our minds

Much of what Cap japan, says seems to be the truth..I have reda the same material and have come up with the same answers. vaginal secretions must have access to your bloodstream..again, assuming no open lesions, cuts, sores etc.., it has to get in by the Urethra, which is the most covered part of the penis..right?

Lets keep talking.

Thanks again for the comfort. I must say that I feel a lot better with some of the comments I've read. Will test at 3 months just to be absolutely certain.

I've had some transient symptoms lately, however, I believe them to be caused by the major anxiety associated with whatever level of "risky" behavior you subject yourself to.

I know not to match symptoms but I have heard that sore throat, swollen lymph nodes, and fever are the hallmarks of ARS. I've had none of these. What are your thoughts?

Let's keep talking.

God Bless.
