Help! BF has low CD4 and high VL

Hi everyone. I'm negative. My bf has been positive for 21 years. He has been non-compliant with his meds off and on over the years. This week, we found out that his CD4 is 65 and VL is 80,000. His doc (an HIV Specialist) is doing the resistance testing but she advised him to stop his HIV meds until he comes back in 6 weeks. This seems counterintuitive and dangerous to me given how low his CD4 is. Does anyone have any idea why she would be doing this?

I am not 100% sure, but I can make a logical guess.

With the resistance test, they need the virus unhindered. They normally perform this test on people before they even start treatment to determine the right course of meds.

Since he has been non compliant and have taken them at his whim, they may not even be doing anything to help. I would venture to say that is the case being he has such a high VL while on medication. Taking the meds may only further compound the problem he is having.

I would also talk to him about seeking treatment with a therapist. It sounds like his non compliance may be a result of depression. Which starting another rounds of meds isn't going to do him a bit of good if he doesn't feel like taking them and will only make matters worse.

I would also ask to go along to the next appointment, I don't think the Doc is getting the full picture, and if she is and doesn't seem concerned about the missed doses.....well find a new doc.

Thanks for the reply. Let me provide a bit more background. His regular doc did a CD4 and VL test about a month ago. That's when the bad results became known. The doc asked if he had been taking his meds (he was prescribed Truvada alone?!?!) My bf re-started Truvada and his regular doc added Sustiva. All of this was a month ago and he has been taking the Truvada and Sustiva for a month (he's scared). I insisted he go to this HIV Specialist. We wen this week. She looked at his labs from a month ago, ordered the resistance testing, and told him to stop the meds that he has been on for a month until the resistance tests come back.

Does that make more sense? Sorry I wasn't clear with my original post.

The original doc screwed up. There is a much higher rate of failure with Truvada taken alone. Chances are he has become resistant to it. So taking the Truvada and the Sustiva together isn't going to do anything but make him resistant to Sustiva as well.

I think the HIV specialist is right on error on the side of caution and stopping the meds till the test is done so he doesn't become resistant to the sustiva as well.

Also look into a therapist, he shouldn't be stopping and starting meds like that and tell the HIV specialist the problems with taking the meds daily. Otherwise you will both be in the same boat again in a few months.

Thanks, kicker, for your responses. I am so hopelessly scared right now. Just having you respond with some answers has helped. I don't really have anyone in my life to talk to about this situation or to ask questions to. You may not realize it, but you've been a lifeline in the last few hours. I thank you so much!

with his counts being that low, any Dr. should have him on preventative antibiotics, until his counts are above 200 at the least.

i agree, that you should accompany him to the Dr visits. dont be afraid to ask questions, and insist on better healthcare, they work for you* sometimes they need reminded of that..

Thanks for your reply and encouragement. The specialist has put him on some OI preventatives. Thanks for encouraging me to go with him and ask questions. That's exactly what I'm doing and plan to continue doing. =) I appreciate your time and response.

I don't understand what "regular doc" actually means but from what you have mentioned here so far looks to me like a very poorly managed situation by someone that obviously has no clue about treating HIV, I'm shocked tbh

It is very important at this stage that you tell the specialist every little detail of what has gone on so far and hopefully they can get things back on track to wellness
