Any False Negative RNA PCR?
      #187175 - 04/20/06 12:28 PM

Please excuse my missuse of this forum. But I really need to hear from anyone who tried to use the PCR to figure out their status and had false negative prior to tesing +. I concerned about the HIV RNA PCR ultrasensitive <50 quant.

no cases reported of false negatives, yes cases reported on false positives. If you have no reactive, PCR you will be find

i dont want to get a contraversy going on here that is not my intention, but from my own experience i had an rna/dna pcr done. they both were undetectable............. i am positive. i have been positive for 7 months now my viral load is still undetectable and my cd4 count is 800.
i will not respond to this thread again, because i dont want to be patranized by the bull crap that people sling at me when i tell my story. pcr's are not used for diagnosis for hiv and are not fda approved for diagnosis. at least last i knew they were not.
take care,
daisey

Daisey,

You're correct...they're not approved for that. In some cases, however, if an AIDS-related condition is suspected, a PCR may be utlized since those tests are faster to complete than a traditional ELISA and WesternBlot test for antibodies....but those cases are usually limited to emergencies where it is imperative that a result be obtained quickly.

But technically your PCR test was correct because your viral load is undetectable and as such the PCR test did not pick it up.

my doc's office tried using it as away to confirm the positive eia and ifa i had already had. when it came back undet. they told me my eia and ifa were false pos. i ended up going and having a western blot done to make me feel more comfortable about being told i was negative. the western blot came back pos, the specialist had to retract her statement of me being negative.
thanks for not chewing on me. i appreciate not having to deal with rudeness.
love,

So if my understanding is correct. The PCR is not a reliable test for determining if the infection took place or not. The reason I ask is that my DR ran one on me 29 days after possible exposure and it came back less than 50. I believe he did it to rule ARS. My limited understanding is that if I was exposed that my viral count would have been high. But reading Daisey's post that is not the case.

SOME people spike a high viral load, some do not. i have not had a viral load yet. my diagnosis was in september 2005. i was exposed in may and june, but did not know i had been exposed. long story short my husband had it, we did not know, we had only been slept together maybe 10 times during that 2 months. we were not married at the time, but we are now. the military found him with hiv during his processing to go to iraq. he is not able to go to iraq now because of his "tainted blood".
at any rate, you should have a blood draw done and an hiv test done, and not by pcr. i went through what we all thought to be ars also. i was exhausted, thrush, herpes of the mouth, sore throat, and the list goes on and on.......... but my pcr was undet.
sorry if this bothers you, that was not my intentions. i just think people ought to know so they can make 100% certain that they are not infected and continue to pass the disease on to others.
best wishes always,
daisey

I am also waiting to be tested after an exposure. I have still a few weeks before i even get to the 6 wk mark.

I have heard that a qualitative DNA PCR has less false positives than the RNA one. Is that true? If I were to test at 6 weeks, which is within the window period, and wanted more assurance than the EIA, would a DNA PCR be of any use?

personally i recommend you choose to be tested by elisa, eia, ifa, or whatever all the tests may be named, but as far as first hand knowledge of pcr's i can only share my own story. if i had of listened to my specialist and went off of the pcr's, i'd be walking around now thinking i was negative. if i was not married, good Lord only knows who i may have infected down the road.

Daisy,
I read your postings on your experience with the PCR's. From my research a false negative is not all that uncommon. I also had a PCR performed, mines was at 29 days RNA PCR<50 and my result was also undetected. Provided me alittle confort but I know that I'm still in the game for a possible + result. This is my 11th week after exposure. My last negative Eli was at the 9 week mark. and I am having symtoms of a viral infection.

The probability of a false -VE turning +VE is very rare. The PCR DNA is used as the standrd in the Adult Entertatinment Industry and they haven't had any false -VEs turn +VE

It may happen but it is very rare

I have noticed on other sites that the PCR can be of some use, but there is a risk of false and negative and positives. Like Daisy mentioned and according to studies by the CDC there False negatives can and do happen. My first mistake was using the RNA Qualitative looking for the viral load<50c/ml. When it is known that there are people out there with VL less than 50 without being on meds. Also while the technology is still evolving its still not 100% for detecting all strains. Most important is that there has to be enough of the virus in the sample.

They have two different types of RNA PCR one more sensitive. I never knew this until I did some research.
I just wonder if the on line labs do the more cheaper one
and charge you $350 HIV IS SO COMPLICATED
