I have just recently been diagnosed with the following labs: CD4 count - 8 Viral load - 780,000 Potentially, I was infected over 10 years ago, but have been relatively symptom free. I was just wondering what percentage of others found themselves in this situation, and how you coped? Also, as regards commencing therapy, I am starting imminently on Truvada + Sustiva - has anyone else started with this in my condition? If so, did it quickly reduce viral load? Did you experience many side effects with it? If so, is there anything you can do to lessen the effects? I have also looked at the possibility of commencing with Truvada + Reyataz + Norvir, which seems to be a popular choice - can anyone advise what they think of this regimen and how they have coped with it? I would very much appreciate anyone's input, as, this is all very new to me and I must admit to being both scared and overrawed. Many thanks, Joe.

Hi Joe and welcome to the boards. I as others know what it feels like. I was diagnosed in July of 05 with a t-cell count of 52 and vl off the charts. There is also aids2hiv who was in the same situation. He will respond soon. I cannot comment on your meds as I am on combivir and sustiva. My t-cells are up to 349 and vl 52. This was in 6 months of diagnosis. I feel great and you will too. Hope to hear more from you.

Hey Fran: My primary care physician put me on combivir for approximately two weeks as a prophylaxis treatment when he learned I was initially exposed to HIV. Then he took me off of it after he learned that that treatment might not work. Anyhow, it had me so dizzy and nauseated that I stayed in a fetal position 80% of the time I was not attempting to work. Did you have similar side effects? I guess I am dreading the idea of medications when I truly have to return to them. Do you finally acclimate to the drugs?

Hey Bhammer. I have been on combivir since day one.(july20) I was first on combivir and tizivir but I was breaking out in a rash. He (my id doc) had to stop all my meds to see what it was but it turned out it was the bactum for the pcp. Anyway, he could not start me back on the trizivir for the problems it can cause by stopping and restarting so he did the combivir and sustiva. I have had the usual side effects from the sustiva which have died out but I have never had a problem with the combivir. I have been very lucky on this regiman thus far. My t-cells are up to 349 from 52 and my viral load is now 52 from off the charts. I know it is scary to think about taking the drugs but they will defenitly find ones to suit you. I think when you do have to go back on them, you will do just fine. Obviously I would stay away from combivir but luckily there are so many other ones. Good luck let me know how it goes.

Hey Fran (If I can call you that?!). Thanks very much for your response. In England, Combivir is now not as popular as Truvada. However, I still recognise the fact that Combivir is a very good, proven drug. By the way, are you aware of any new drugs in the HIV pipeline, particularly from new classes (New drug information seems to be more readily available in the States than in England)? Cheers, Joe.

Thats a good combo, of meds Joe....last year when i tested and was diagnosed, my tcells was 15, and my viral load was so high it couldnt be charted. I also had some oppurtunistic infections, that complicated things and laid me in bed for 3 months...hwoever once they started me on the meds.....my tcells went up, my viral load went undetectable within a relatively short time....Your Doctor should put you on some anitbiotics to help ward off OI's,anytime your Tcell drops below 200. You are very susceptable to them at that point* Good Luck, God Bless, and welcome to the boards....

i'm sorry Joe, i did want to say, as far as the meds go, everyone reacts differently to them, some have side affects, some don't. It's just a trial and error thing, i suggest you and your doctor build a strong communication on your personal health as it is vital to the quality of treatment....God Bless*

Hi Joe.. Welcome to thebody. Any one of those meds will kick the hell out of your VL and drop it quite quickly. CD4's will take a bit of time. I would hope that your IDS is putting you on some meds to fight any infection until your body takes over on these to regimines. I was offered sustiva but opted out because I am a "Mental case" and it would get me all uptight and freaky. Not that I could not take them. I just want a stable brain function until I can learn how to cope.
Congradulations on being meds free for as long as you have. Thats a long time. Now its time to give you body a break. And get back what you have lost. The meds that you talked about are state of the art in the US. I am also hearing rumers of a Phase 2 PI coming out in not to long. But I have no details as yet..

Is sustiva a drug that fights hiv but makes you go crazy mentally? I seen others here taking it and saying it works well for them.

It does not make you go crazy mentally necessarily. The first week or two it can effect your dreams and you may holucinate. I have been on it for 7 months now and that first night was WOW!!!!!! I could not beleive the dreams and holucinations I had. After that first night I was fine. I still hae some odd dreams now and again but nothing that I can not tolerate. It has been working very well for me.

Well the situation of just finding out after 15 years no, I found out in Nov 1989 after I went through a tremendously hard time (father dieing June 1989 while I was away at work in Europe) and did some stupid things sexually while there and before I came back home for the winter off season. I tested and voila I was poz, woohoo I figured there go’s my career and everything I have worked for (needless to say it didn't) I went through all the normal stuff finding a person who had been one of the biggest supporters in the early 80's still standing by my side telling me he would always be there and he would make sure no one ever knew and could tell because we were going to keep my body so strong the disease will never have time or the ability to cause me to get any illnesses (he died in 2002 after being hit by a car on his bicycle), I continued in Europe until 1994 when they finally retired me because of age and not anything physical. In 2000 I had a little physical setback due to a big time auto accident and major spinal trauma which is the only thing I think that would be a denominator in causing my physical and mental state to get to a point of having to do too much work and allowing the HIV virus do it's job and start doing a number on my system, I watched my T-cells go from the high 700s to 300, then 200 then 100 the whole time my viral load staying in the 8 to 200,000 range. I finally started meds (Combivir and Sustiva) Jan, 2004 without an opportune infection or an Aids related illness with a T-cell count of 5 and a viral load of only 152,000. Since I started meds I went from 5 to the point I am now which is 482 and un-detectable. Don’t worry too much being treatment naïve that is the best way to start with numbers in that renge, that means you will have many many years of options before the combos run out if you have to change and the new stuff coming in to the system every year pretty much insures that if there is a combo you can not tolerate you have many you can switch to. Just help your self a bit by attempting to keep a good attitude ( I know it is hard but I have been doing it for a long time) and a little bit of looking out for your body makes a big difference, I still try to keep as fit and active as I can even with the cane and the Dec 8, 2005 three level cervical fusion ( my pic) but I am on my feet, trying not to let the depressions and all the good food get me down, maybe the end of march I can start therapy and be allowed to drive again so I can get out and about and work on me some more. I hope you just relax, and work on you.

I'm on Sustiva and Truvada. I'm quite happy with them. The once daily dosage and two convenient pills is great. The Sustiva rarely gives me any weird dreams, and my viral load has been undetectable for the last year (took about 2.5 months to get there).
Another nice thing...the combo pill that will put Sustiva and Truvada together into one tablet is expected to be approved by the FDA later this year. If it's approved here, I would imagine it will be approved in other countries as well.
I can't wait...one pill once per day.

I had the option of Sustiva, but the doctor wanted my mental state to get better. I beleive there is a warning about depressional side effects. I imagine once I get this hammered out, If I have to switch someday that this regimin would be one of them.. I like my brain the way its not.. No need to add anymore weirdness to it yet...

my first set of labs came back when i was diagnosed 8/23/05 with my CD4 280 & my VL 86,000. My doctor recommended I start meds (SUSTIVA & TRUVADA) i started 10/3/05. 2nd labs (28 days later) 10/31/05 my CD4 330 & VL undetectable. My 3rd set of labs 2/16/06 VL undetectable & my CD4 is 651! My doctor told me he'll see me in 12 weeks, get out of his office because his office is for sick people. He told me to go live my life... but be careful & take my meds! He said that there is nothing wrong with me & I am perfectly healthy (minor cold, but I'm all over it now)
