When do i start treatment????

I am 23 and have recently been diagnosed with hiv about three weeks ago. I am located in Australia and am working closely with my doctor and dietian ... My query is, do the treatment guidelines change country to country.... my cd4 is 530 and viral load 75 000... If i was being treated in another country would i have started meds yet????? Alot of info says yes but i think it is out dated... very scared but keeping my chin up!

don't be scared. work closely with your doctor and dietian and things will be fine. you have been only diagnosed for three weeks (not sure if that means you were infected within 3 month period or ony discovered your positive serostatus) so you need more data points before any major changes. so for you cd4 count is good. you could also look up info at aidsmeds.com they have very good information and lots of experts who regularly check the website (unlike this one). good luck

When to start meds is the same no matter what country your from. And your not going to like my answer but its a choice that only you can decide. Talk with your dr. and you can talk to some very knowledgable folks in the delphi web page for aids. Hope this helps a little

You are smart. The cutting edge doctors would probably not treat you at this point. Your VL might grow slowly, or even drop down on its own. Since we are not from different species, the best ways to approach HIV treatment, should vary according to individual parameters, not continent. Unfortunately, countries, cities, and rural areas, all tend to treat HIV differently. I would suggest you find the best doctors or medical research center, and if none is available, at least read up on what they would be doing, if you were a patient (virtual treatment, lol).
Here's my suggestion; get another specialist. Second get all your blood work done again and look at CD4, CD8, and viral loads (parts/ml). Don't use diffenet labs each time. They will give very diffenent results from the same blood (I know!!) If your T cells are high, and your viral load somewhere still below 100,000, you might simply monitor your numbers (every 3-4 months). If you are higher, or suddenly spike, or T cells half in number it might be a good idea to start treatment. Here's my medical history. I was infected at least 15 years ago. I started treatment 10 years ago on 3tc and d4t (an unusually mild concoction I read about in an HIV magazine). My pretreatment VL was 43000 parts/ml. After a few weeks my Vl dropped to 50 to 4000, and my perfectly fine T cells went from 750-1000 to 1100-1400. After five years treatment, my specialist MD, asked me, and most all her healthier patients (several thousand!) to quit treatment (STI) indefinitely! That was about five years ago. My VL is between 1000 and 4000. My T cells seem to have dropped from 750 down to around 475 or 500. I am not overly concerned about these numbers, and astounded that my situation is so stable years after infection, and years after suspending treatment. I worry about heart disease and prostate cancer more than imminent death from HIV. I don't know what lessons you may take from my experience, but rest assured monitoring without treatment is not foolish. It might be a wise choice for you as well. Should you need treatment, I have no suggestions as to what drugs to take--there are better choices now than when I took mine. Best of luck!

Hi. I'm sorry to read your pain. To answer your question, in Canada the "key" numbers before going on meds are CD4 200 or less and VL 100,000. I am not on meds yet. Educate yourself and remind yourself that it is no longer a "death" sentence but a manageable disease. You will be fine.

Now to me, that seems extreme. We tell most folks with a CD4 fo under 350 and VL above 55,000 to go on treatment. Many docs prefer to wait until the CD4 count is that low, even if the VL is higher, for fear of premature resistance. CD4 count of 200 or less is AIDS-defining, and I don't think it's a good idea to wait that long.

I would like to thank all that have replied to this query as it has eased my mind to read each and every response............. The lessons learnt from all replies have made me get more bloods done and sit pretty untill further notice. Also I now have a team of medical experts, sports coaches and supportive friends to help me face this head on! This challenge will test my courage and dedication to self... AND I ALWAYS WIN!!!!
