I need to choose my first treatment for HIV. Iwas diagnosed last year. Probably I have it for five years. I need to start thinking of treatment as my cd count is around 250 and I don't want to wait too much longer. My viral load is around 40,ooo. I was wondering how did people decide which treatment. There are so many choices and new drugs. My doctor is great but asks me what I think and I want to be able to respond intelligently.

If I was starting today I would try quad therapy--Combivir, Abacavir and Efavirenz. There was a study about it and 90% of people got an undetecable viral load by 48 weeks...But make sure to get a resistance test before you take any drugs...just to make sure you are not resistant to something...no point trying and failing.

I started with Combivir and Sustiva, my basic reason for this was that it is sometimes an over looked class of meds that show very good results. My VL went from 150,000 to undectable in 90 days after starting the meds and has remained undectable for 4 years. My CD's have gone from 250 - 875 in the same amount of time.
I also felt that by starting with a lower class then the PI's if anything went wrong I would have more classes to explore.
I'm not going to say that Combivir and Sustiva are the easiest, because the Sustiva does have some side effects, mostly involving sleeping and some people do have a rash, but I take 2 pills once a day before bed which is very easy to remember.
Good Luck on whatever you choose
