I have had recurrent extreme fatigue for the past 21 years. I have neither anemia nor parasites, eat and sleep well, not feeling depressed nor stressed out, taking vitamins. What could be causing this, and what possible recourses to diagnosis or treatments are available

HIV itself especially untreated can make you tired. Are you on medication?

Try looking at where you are giving your power to. Are you in a job, relationship, place where do not want to be. Our energy comes from more than just physical forces. We lose our energy by being victims or being around people who suck our energy (sometimes called "phychic vampires"). This is serious stuff and if you have looked at the physical side and came up with nothing try this metaphysical approach.
