Persistent Sinusitis
      #133387 - 01/23/05 01:38 AM

Hi there....I hope everyone here is doing well and having a good start to the new year. I've came across a lot of good people on this board and I really could use some opinions on my situation, realizing that it isn't necessarily "medical advice." This might be a lengthy post but please bear with me. I'm kinda stuck in a rut right now and I'm wondering if anyone else can help, or, has been in my situation and what they have done. For the past 6 years or so, I keep getting constant sinusitis, which gets successfully treated with antibiotics, but then recurs every 2-3 months. This happens despite having an undetectable viral load. Right now, I'm really, really sick from sinusitis, which has turned into a combination of bronchitis, as well. When I sleep, my infected sinus secretions drip down my throat, which then causes bronchial congestion and bronchitis. Usually, when I get sinusitis, this "almost always" happens to my lungs. I've been coughing and coughing so hard, that my back muscles hurt now. I've drank hot liquids which temporarily clear my nose, but then again, within an hour or so, my nose is completely blocked and I'm coughing so hard that I'm in pain. I'm wondering if I should see an ear/nose/throat specialist and get something done. Does anyone else here have these constant/frequent sinus/bronchial infections? At one point, when I lived in CA, I started getting gamma globulin infusions which really helped. Now I live in NYC and the bloodtest for my gamma globulin levels shows up normal, so Medicaid won't cover it, but I know it would help. I'm scared too. I've had HIV for almost 15 years now. Sometimes I think that maybe the end is coming near because I also get apthous ulcers in the mouth a lot and I read a study that associated having frequent apthous ulcers with increased morbidity in HIV patients. Anyhow, I don't know what to do. I'm kind of ashamed to admit to you all that I'm scared because I've been so strong emotionally with HIV for many years. I feel like I'm falling apart. There has to be someone who is in my situation with both the frequent sinus problems and now, my new fear. Sorry to have gone on for so long. Anyhow, everyone take care of yourselves and any input would be very appreciated.

Post your question in the side effects forum
http://www.thebody.com/Forums/AIDS/SideEffects/ask.html

and also ask other doctors in the forums. I would get a referral from your hiv specialist to an ENT specialist who deals with HIV positive people. You want something that will help you. It sounds miserable. If you are undetactable that's great...lots of negative people have these problems... It may be unrelated to HIV. WHat's your tcell count?

I do have sinus problems, HIV+ for over 14 years. It's been a constant battle. Sent to ENT who could only find minor abnormalities. I do have nasal allergies and on an inhaler, my dr said there is an actual condition for when allergies cause sinus problems, which is part of my problem. I used to getat least 6 infections a year, down to 2, maybe 3. Don't know if you have allergies, I found trying to avoid mold/mildew (live in a damp climate), and cigarette smoke (dries out nasal passages and sinuses). And keeping things halfway dusted. I have found some things that work...the DR. Grossan (there may be others now) nasal irrigation system is WONDERFUl. No pain, it is about $100 to buy, but I've had mine for years!!! I also take an expectorant (say Mucinex) , I take one a day for preventative, more if feel a bout coming all. Also run vaporizer for 2 hours or put a steam pot in my room for moisture. I bought eucalyptus oil and it helps clear up passages, you can add to steam, etc. Unfortunately, from what I've read, many people with HIV have sinus problems, they don't know why. But there are alot of things you can try....I was at the going crazy point! There are some sinus websites, I might try "Sinus Buster" that has capsacin in it. Some good books too, Dr. Murray Grossan is an expert in the field, I think he has a site also. Some sites like Allergy Buyer's Club have info on sinus problems. Good luck!

I've had problems with chronic sinusitis. I used to get infected 3 or 4 times a year. I did a consult with Lark Lands( http://www.larrylands.com/lark/) she told me to go to the store and buy "saline nasal spray",(it's cheap). I spray each nostril 2 times, 6 or 7 times a day. It has worked great for me, haven't had any sinus trouble in over a year. Try it, it might work for you as well.
As far as apthous ulcers, I have not had them but maybe you should see a dentist or oral hygienist.
How are your numbers? (VL and CD4)
Don't be ashamed to admit you are scared, I think we all get scared now and then. When you are sick it makes things worse. You have been fighting this for 15 years now, DON'T GIVE UP!!! YOU WILL GET THROUGH THIS!!!!

This may seem like its just too simple but people have been dealing with this condition for ages by using a saline solution (salt and water) treament. You can go to pretty much any major department store or drug store where they have toothbrushes and such and look for a pump unit that has a container and hose on it usually used to clean your teeth. Mine was made by Interplak. Go on the internet and look for instructions on how to treat your sinuses with it. In just a few days your condition should clear up. Hope this helps.
