Acute HIV Infection - Help!!

I got fever 4 weeks after a sex worker gave me unprotected oral sex. The fever was 99F with headache, weakness, night sweats, funny sensations in my body, tingling in my hands and feet. This fever has lasted 6 weeks and aint going. In the 2nd and 5th week I got CBC done. High Lymph >50%, low neuts <35%, abs wbc 8000. sed rate <10. I then got a p24 - negative, elisa - negative (67 days after close encounter and 30 days into the fever). Lymph subset normal with normal CD4 and CD8.. Am I going to die? Do I have it? how do I proceed?

you dont get hiv that way. Go see a psychiatrist urgently!

99F is not considered a fever. It's within the normal variation of people's temperature. ARS symptoms DO NOT last for weeks at a time.

No one has ever been infected from getting a blow job and you have had a negative p24 and negative ELISA. Even though you have not yet taken a conclusive 3 month ELISA with the P24 you can be assured you don't have HIV.

your tests say negative - you are negative and you dont get hiv from blowjobs.
but you will live on the edge untill you run to test at 3 months - so try to calm down untill then and ......
wear a rubber next time and stay away from hookers!

If this is your only exposure, you are more likely to win thelottery than catch HIV. But as someone else said, stay away from the sex workers, that is my opinion. It will probably save you some money and heartache aswell.

(Reports 5 cases of oral transmission in France. 2 cases attributed to insertive oral sex, 1 case to receptive without ejaculation, and 2 cases to receptive with ejaculation.

Thank you for replying to my post Jackie Blue. Could elevated lymphocytes, lowered neutrophils, sed rate of 9 and these symptoms at all be acute HIV? I know I'm being stupid and was stupid when I indulged in such risky activities but I'm having a torrid time waiting for the 3 months. Thank you for the help.

Your symptoms sound far more like stress than ARS. In fact as someone has pointed out, they sound nothing like ARS.

And don't say you weren't stressed when the "fever" came, because you obviously were. Otherwise you wouldn't have gone having CBC's at 2 weeks.

Anyway, your tests strongly suggest you're negative. Negative p24 and negative ELISA at nearly 10 weeks means you should feel confident.

Plus your risk was very low. I've never heard of the report someone quoted but note it was from 1988, 15 years ago, that's a long long time and all I see from doctors now is that receiving oral sex is just a theoretical risk, meaning it is possible but hugely unlikely, and meaning that doctors never see anyone who has got HIV from receiving oral.

Everything seems to be in your favour. It's hell to feel so sick for so long, especially with the worry about what might be causing it. But you're close to a 12 week test which will be conclusive and which I'm sure will be negative.

Don't panic - Oral Sex transmission to insertive partner in fellatio is possible though not probable. The statistical chances are minute and involve the possibility of infection that causes blood to mix with blood in saliva (it seems like an impossibility!). However, it has been recently found that SIV from the saliva of monkeys though 6000 times less in load is much more likely to cause an infection statistically in terms of infections per number of viral particles - meaning these viruses are probably more virulent. I don't know how far this is true and I don't think it affects you but its always good to wear a condom and not have sex with commercial sex cowrkers. Remember - only now are we really finding out if oral sex can be a route for transfer of HIV - because people seem to be more careful with penile-anal and penile-vaginal sex - so please keep that condom ON.

Blood work does not indicate HIV infection. Only an antibody test can do that. Having an elevated sed rate only means you have an infection or condition. It doesn't indicate what type of infection or condition. My dad's sed rate was once high, which sent them looking, they discovered he had Prostate Cancer.

Fellatio -no chances either receptive or insertive new

Its not possible to get HIV via fellatio. All reports associated with this acitivity that support such transmission are inadequate in detail, may not be the stated reality (because humans are known to give fictitious information).. Even the doctors who carried out the study have agreed that there maybe insufficient data to support the claim. I would really like to see a single, detailed reported case of HIV transmission through fellatio!!

Heres what Dr. Bob at The Body says in reply to a question on the issue:
Hi,

Well actually the final verdict is still out, but my personal opinion is the direct opposite of yours. The risk of HIV infection associated with receptive oral sex (fellatio) has always been a hot topic.

Early studies found no independent risk for giving or getting blow jobs; however, the high correlation of risk with multiple sex partners raised the possibility that a risk existed, but could not be detected. Subsequently, there were anecdotal reports of possible HIV infections from men who deny other risk behaviors. Consequently, the prevailing wisdom was fellatio appeared to carry a very small, but not non-existent risk. Most safe sex guidelines list unprotected oral sex as "low risk." A study last year (AIDS 2002; 16:2350-2352) reported that 8% of HIV-positive participants acquired HIV from fellatio. All hell broke loose following that report, because it was interpreted (or should I say, "misinterpreted?") to mean that 8% of HIV infections among men having sex with men are attributable to receptive oral sex. That's not the case. I can't go into great detail about statistical analysis, but to understand this study, one needs to understand the PAR (population-attributable risk percentage). The PAR percent can explain how a very low-risk exposure could result in a substantial proportion of infections in this type of study.

More recent data confirm that oral sex carries a very low risk of contracting HIV. In San Francisco, for instance, 10,283 clients were evaluated, of which 413 (4 percent) were eligible, and 243 (2.3 percent) agreed to participate in a study to evaluate the HIV risk from fellatio. The estimated probability of orally acquired HIV was found to be zero, because no recently acquired HIV infections were detected. These folks were all men having oral sex with men. The median number of oral sex partners within the previous 6 months was 3 and 28 percent of the oral sex episodes were "unprotected." One third reported getting cum in their mouths and of those, 70 percent swallowed it. Oral sex on a known HIV-positive partner was reported by 28 percent, and 39 percent of these guys swallowed the ejaculate. Again, no cases of HIV infection were found. OK, back to statistics. This is a small study and certainly does not completely rule out the possibility that HIV can be transmitted by receptive unprotected oral sex. It does, however, give considerable weight to the argument that the risk of acquiring HIV from oral sex is extremely low. The take-home message here is really that one's choice of sexual practices does matter when it comes to HIV risk.
