Virus Trapped in Lymphatic System Follow Up

Dear all - thanks for your responses. However there is a number of entries on the Body from people who have suffered from Persistent Generalized Swollen Nodes and been negative on Elisa and PCRs but eventually seroconverted - see below for instance;

Question:

Dear Dr. Cohen, I am the feloow who seroconverted after nine months and had PGL for six months before completing seroconversion. My VL which had been undetectable rose to 80,000 and my t-cells went down from 1065 to 750 in one month. How can this be?? Everything was in a delicate balance until I developed a real bad cold and bronchitis right before turning positive. I guess this infection shook the system and I seroconverted. Do you have an explanation for the drastic change??






Answer:

I don't have any easy explanation - no more than what you mentioned which is that HIV was quiet until the infection from the bronchitis just shook the immune system into high gear and triggered HIV replication as well.

The question is what happens to your viral load when the bronchitis is gone - will it go back down to a low number, or stay 80,000? You should know about four weeks or so after you recover from the bronchitis - that is when we see people recover and have a viral load go back to a 'set point'.

And if your viral load is now 80,000- the news is that since this increase in viral load is very recent - it may be that hitting early with antivirals allows you to maintain the health of your immune system by preventing the loss of your immune function - especially the part that controls HIV itself -- so far - this has mainly been shown when we treat within the first six months after exposure. Which is not the case for you, but since HIV was quiet for the first several months it may apply to you as well. It is hard to know for sure given your story - but a consideration in deciding whether to treat now or later.

So - you need to consider your options - and whether there is a regimen you would be comfortable taking. It doesn't mean you need to stay on your first regimen for life - just one you can take for now. We have reviewed these issues on the site many times - and it means knowing what the different meds demand of you, and what you are willing to commit to, if any. And establishing a relationship with a provider you can work honestly with.

What he says is possible I guess, but I'm surprised we haven't seen a case study on this person. What the person is describing is Acute infection. Acute infection doesn't go on for 9 months. My guess is that this person had other exposures and the orginial one the 9 months before has nothing to do with the fact this person is now positive. The fact they had negative Elisa and PCR, yet went on to do viral loads and cd4 counts doesn't really make any sense to me. I don't know why any doctor would do the other tests in absence of a reason.

Because anyone can write anything they want on the internet and we can only take it at face value. How do we know there were no other exposures? Why would someone who is negative have VL counts done. Maybe what he thought was bronchitus was actually ARS. Who knows. Was he continually testing up until the 9 months? or did he test within the window and then not again until 9 months?
and lastly that post is 4 years old and was highly disputed at the time.

Let's start at the beginning. Did you have an actual exposure?

Details of exposure, symptons etc new

I had a unprotected exposure with someone of unknown status. 2 weeks later came down with swollen glands, sore throat, hot/cold flushes, diarreah, night sweats. All symptons have gone except for swollen glands which continue to this day - 29 weeks post exposure. Have been tested for EBV, CMV, Hep, Toxo etc but no diagnosable cause. White blood count has been continually down throughout the whole period with lymphocytes elevated. Hence doctor says defintiely a viral infection - the question is what viral infection? Whilst the PCR has continually been undetectable I wouldn't be suprised if my lymphatic viral load would be sky high??

Can you see why I am concerned or do you really think that something would have shown on these tests??

Your advice/ideas etc is really really appreciated. Thankyou
