Firstly I would like to thank those who responded to my Ďlatest update postí a few weeks back. In response to your questions (sorry I didnít get back earlier I went on a 3 day bender and consequently have only just learnt how to type again): -

Jackie the reason I donít think I have an undetectable worm/parasite is that I have probably spent 2 years of my life holidaying in places like India Thailand and Africa during those times I have had motorbike accidents, I have been bitten raged by mosquitoes, broken my foot and had unimaginable food poisoning. That last time I went to Thailand I had no accidents and was not unwell but I did have sex with prostitutes something I have never done before then I got sick. By the way I am a huge fan of yours please donít leave the for ham.

Xander thanks for the eosophile link yes raised levels can mean many things apparently quite often it indicates a worm or parasite infection but if you do a search with ĎHIV raised eosophilesí you will see that people with HIV quite often have eosophilia.

So just to recap and to
Inform all you new worriers this is the letter I sent to the doctor at the Hospital for Tropical Diseases

Dear Dr

Firstly I would like to thank you for the time you have spent on me in search of the cause of my symptoms. It appears that after some pretty exhaustive tests that I do not have worms. This leads me to believe that it is necessary to follow up with further investigations as regards to HIV, I am convinced that I have a new strain of the virus which either creates antibodies that are not picked up on the currant ELISA, or I am not producing antibodies.

As regards to my exposure, which accured on 29/01/2001: -

First exposure I had a threesome with two sex workers, which included protected vaginal intercourse but unprotected receptive oral sex after the two girls had performed oral sex on each other. One of the girls said she did not mind if I had unprotected sex with her.

Second exposure was with a sex worker I had protected vaginal intercourse with her but the condom did slip off completely at one point. I also performed oral sex on her.
She was at the end of her menstrual cycle I found out later, she was not bothered if I used a condom or not and she had what I would describe as Karposis Sarcoma over her stomach.

Sorry to be so graphic with the above but I think it is important that you know just how risky my exposure was.

Just as a recap the following is a list of symptoms with duration: -

3 weeks started to lose appetite lost 2 stone appetites came back after 4 months I am now back to my normal weight. (A bit fat!)
4 weeks severe non-stop diareah 4 months
4.5 weeks slight pain in lower legs
5 weeks severe pain in toes & legs especially front/back of knees
5 weeks glands in armpits swollen this continued on and off for the next 8 months.
5 weeks drenching night sweats lasted 2 week. I still sweat today especially soles of feet and palms of hands. Now if I walk for more than 10 minutes sweat pours down my face, also eating sometimes makes me sweat.
Also about this time ear infection felt like something crawling around in my ear! Lasted 2 months.
6 weeks noticed white coating on tongue still present today. Also felt a painful lump on the back of my tongue lasted 3 months
6.5 weeks severe muscle & joint pain had raised to my hips, back, arms and fingers.
Severe pain lasted for 4 months could not walk, as knees were especially bad, I still have pain now although not as bad. I always have pain somewhere every day, which can crop up anywhere but mostly legs, toes, fingers and arms, I have also had lower back pain constantly for the last 3 months
7 weeks prickly burning sore sensation in legs still present today I can only ware tracksuit bottoms, as other trousers are too uncomfortable
8 weeks legs swell up so bad they split open my trousers! Still feel tight today
9 weeks skin starts to peal off my palms lasted 1 month
5 months vomiting after food lasted 2 months
9 months muscle twitching in various places lasting up to 3 weeks at a time better now only happens once in a while also vibating feeling in muscles.

Other miscellaneous symptoms
Floaters black spots and lines in my vision very apparent
Also blurred vision
Herpes 1 and 2 outbreaks much more frequently than before
Shortness of breath
Fatigue.

My tests so far: -

6.5 weeks ELISA
3 months ELISA
5 months ELISA
7 months ELISA
8 months HIV viral antigen load
11.5 months ELISA performed in Thailand (test turnaround time 4 hours?)
All negative

Tests I feel I need: -

Western Blot assay for HIV 1 & 2, which can pick up the N subtype, please read info I have enclosed it appears that this subtype does exist in Thailand

bDNA PCR this viral load test picks up non-B clade strains, which I may have

CD4 count (if itís within normal ranges OK, if not I need to know)

Chest x ray

Possible visible detection by electron microscope

ELISA fine-tuned to react with rare subtypes

Please do not miss intemperate this letter, I am not questioning your ability as a doctor of which I am not but I have been researching my symptoms for more than a year spending many hours a day on the internet. I have not bean reading out of date literature only legitimate reports and consider myself quite well read on this subject.

You may think I have lost the plot. Yes I am seeing a physiatrist who thinks my symptoms are caused by stress, I can assure you this is not the case. Although I can see that without the pain that I feel an onlooker might well think this is all in my head.

Well thatís about it. It did cross my mind that it could be the expense of the tests that are making you reluctant to run them well if that is the case I am willing to pay for everything, I feel time is of the essence because of the duration of my symptoms I am probably going to progress to AIDS relatively quickly and there for will be requiring medication soon. Of course this is not a diagnosis I want and if I had no ongoing disabling symptoms as I have I probably would sooner not know. Please help me, you are my last hope I donít know where to turn next.

So I went to se the Doctor and after a 2 hour conversation she prescribed me some worming tablets (I havenít taken them yet but how good is it going to feel if I feel better after I take them?) then she said she had spoken with a HIV specialist who is willing to do a test that looks for the env, pol and gag parts of the virus which I always thought was the purpose of the Western Blot but she assured me that the test was not a WB? (A new test maybe, I thought I new about all the different tests. Sounds like BS to me.)

She is also testing for Diengy fever, hep B C doing a CBC, and at last a CD4 count, I have been pushing for these tests for over a year now so you can imagine my relief when she agreed to run them. If any body out there disagrees with me because they think I am waiting recourses on unnecessary tests you can ****of, I am positive and I know it I just want medication and support like anybody else.

Oh and while I am in a bad mood I will point out one other thing to all you Americans out there, try to think outside the box a little sometimes, while I was last in Thailand your Marines were not just getting manicures in Bangkok. Think about it, Marine ****s prostitutes he comes home ****s you, when he is on duty you get bored, you **** some one else, yes there are probably many many more undetectable strains in your country than you are lead to believe.

And that my friends is my bottom line

i understand what you are experiencing but it is unfair to drop bombs like the "marine fu_ks prostitute + marine fu_ks you = mega hiv group xyz contamination" paradigm on this forum. Let's try to be less catastrophic.

I could say that here in italy prostitution is mainly african so hiv 1/2 testing is mandatory, but who knows about subtypes? Africa is the region with the greatest number of strains and subtypes and almost every 5 years a new strain comes out. however i confide in my tests. Am i dumb?

In my opinion I would consider myself dumb to confide in my HIV negative test result if there was no other diagnosis to explain my symptoms, which are clearly not in my head. Another point I would like to make is that in Thailand subtype B used to be the predominant strain but was soon superseded by subtype E which is much easier to contract heterosexually and has spread very quickly around that country so in my mind it would not be that unusual that it could spread with the same ferocity in another country that did note have that strain before.

The Internet doctors are always responding to my questions by saying ĎThe predominant subtype in Thailand is subtype E which essentially would show up on conventional assaysí I underlined the words that make the statement a little wishy washy. I have read that when you look down the subtype tree when you get to a branch of a branch the tests become less reliable, I have also read that some subtypes in Asia can escape the ELISA. And the cherry on the cake is that the strains in Asia are more virulent often leading to death within 2 years of exposure.

Why are you on this board what are your concerns?

I totally agree with your opinion. If you think you're HIV+, you most probably are. I tested negative at 3 and 6 months. I will test again at 9 months (in two weeks), but no matter what the result is, I know I am + because I have not felt the same for the last 8.5 months. I know I am not normal and I know when it all started. Coincidence? No way.

Springer subtype O is way down the branch yet the gale who was infected in 1996 with this very rare subtype at least had a reaction on most Elisa tests. What makes you think the test wont pick up the other subtypes? Im sure after 9 months there would have been at least some kind of reaction with the test. Am i wrong?

My exposure: three unprotected vaginal intercourses with girls i don't know well.

My symptoms: not very specific (sore throat, fatigue, severe malaise, myalgya arthralgya, one submandibular node, no fever, no rash)

My fear: paranoia

My tests neg hiv 1-2 elisa + p24 at 3 and 4.5 months, pretty safe, i'm quite out of it

My project: to rule out the incredible i'll test at 6 months. I am not going to do any pcr or cd4 count or other strange test.

Hi Springer:

I can totally relate to your situation as i do have the same problem as you do.

I had unprotected sex with a female prostitute in China August of last year(so it has been 8 months now). I tested negative on 2 elisa test in month 7, negative or non-detected viral load ie; less than 50 copies. I was again tested negative 2 weeks ago on both elisa hiv-1 & hiv-2.
I have the following persistant symtoms for started 2 months ago and they are still on-going:
-lost 10 pounds in the past 2 months
-chronic diarrhea
-fast heart rate especially after meal
-red dots(lessions) all over my trunk and my arm and they are not going away
-major fatique(i have to sleep 10 hours a day)
-low appetite
-dry skin, dry mouth and lips
-blurry vision(i had good vision prior to the exposure)
I also have other symtoms which i do not want to list them all.
I called the cdc and have also talked to the local hiv councilor and all i was told was that there are only 2 documented cases in the us that the patients seroconverted after 6 months and my exposure was 8 months ago and therefore i am hiv negative.
Since i am getting no help from anybody i would just have to pursue it on my own. I have just taken the bRNA to detect other strins of hiv and i should be getting the result in just couple of days. i am sure i am positive because all these are symtoms that i have never experienced in the past. But i guess nobody will trust me since test result means everything to them and that those on-going symtoms are just meaningless.

Anyway, i wish you the best of luck and hoping that everything is going to work out fine for the both of us and i will continue to check out your post on this forum.

Itís not O subtype I am concerned about it is N or perhaps some other strain that has not been discovered yet. The tests in the UK have been able to react with the O subtype for sometime.

Hi Springer, I've just come back from China (Shanghai) a few weeks back. Whilst there I started to have protected sex with a girl I picked up in a bar who pulled my condom off midway through it. The next morning I discovered that she was also having her period.

I felt my lymph nodes in the back of next rise within 12 hours and had mild sweats for a couple of weeks. Thus far, everything else is normal, but I am still crapping myself over the direct blood exposure - I could see it on my todger.

Are you okay now - Have your symptoms sorted themselves out ?.
