when they talk about 2 or more extra inguial sites have swollen nodes whats that mean

It means swollen nodes in multiple locations. Inguial is your groin area, there are nodes in your armpits, neck, behind ears and other places. Unless you have swollen nodes in all areas mentioned you have little to worry about.
