After a night of drinking I recently hooked up with two guys who were drug users. I had unprotected sex with one (I was the top), performed oral sex and rimming on both, and fisting on one. I have no idea what there status was (I made a very stupid mistake). I have until tomorrow to decide on PEP. I am not sure about my risk factors and whether or not I should do PEP. My appointment is scheduled for tomorrow, but I am not sure if it is necessary or not..

unprotected sex is a risk for HIV, though being the top is less risky than being the bottom. according to the cdc, for unprotected insertive anal intercourse with an HIV-positive partner, the per-act risk estimate is 6.5 per 10,000 exposures.
oral sex is very, very low risk. about 0.5 per 10,000 exposures.
rimming and fisting are not considered risks.
and keep in mind, these estimates are for partners who are confirmed HIV positive. hopefully, the two guys were negative.
PEP is usually reserved for occupational exposures or significant encounters with a person who is confirmed HIV positive. And PEP is only effective if you start within 72 hours after exposure. you have to stick with it for 30 days, and of course there are all those possible side effects. just things to consider. definitely talk to your doctor and see what he/she recommends.
but don't be too hard on yourself. these things happen. let's hope you're still negative. in the future, be more careful.
