Testing methods abd CD4 count question.

I am going to have another test next week. But here in Ireland the only tests available are ag/ab blood analysis. They only do a PCR test if they find a positive result from the initial test. I dont know what to do. I feel so restricted. And I dont understand what an RNA?PCR test is. All I know is that I am terrified, and that I feel like a sticky bun combined with a lump of sandpaper. (yeast infections, dermatitis on scalp, sweats, sticky skin) I pray for a return to good health. And why would my CD4 count be only 540 if I was hiv negative?

That's some amusing descriptions , let's see if we can assure you of your most likely negative status . Go to "medhealth.com" click onto the" ask the experts" Hiv International forum , questions there are answered by 3 Hiv docs in one of Englands largest clinics, I believe the test you are about to take : the ag/ab blood analysis are highly dependable and accurate at a very early interval (6 weks , maybe 4 ???) and are often referenced by the Docs on that forum . We'll get to the RNA test only if you get a positive (Which you won't ) . CD4 counts vary for ANY infection , not just Hiv , yes you have something going on but it's very early for your CD'4s to be low from an Hiv infection this soon , so relax on that count . Whoever gave you that test ought to be slapped upside the head with the report . You know , myself and other post answeres have asked you some definitive questions about your supposed Hiv exposure and you've been vague at best , what gives ? Where is the PROFF !! that your partner is positive , and I do not mean someone told you baloney , ASK him, or ask him to get tested !!! Does your local health board keep names of known infected individuas ? What is the EXACT date of your encounter with Mr. supposed ? How many times ? Answers you'll get here are limited to the info you give , so what gives ? It seems you have some means to get accurate info but are not making any attempts to do so ..? Even a "girlfriend" to come on to him and ask him ...? Or have him prove it to her by getting a test , trust me there's many things we guys will do for new pun, pun !!!

How do I know that he is positive? Ok. I'll give you the details.
It was 2nd November 2009, it was a one night stand (something I have never done before). I asked him if he had any health problems, he said no. Afterwards he avoided me, would not take my phone calls, so I put myself in a place where I knew he would be, and asked him if he was HIV positive, he said "No, you can ask my GP if you want" ... in the UK...where he lives GPs dont have records if a person is pos or not (its all confidentail and at the gum clinic). Why do I say I know he is positive? Well lets say I am 99.9% sure that he is. Because: he used to be thin, now he is really fat, all the fat is on his tummy (he looks nine months pregnant) and his legs and arms are thin. Plus he had pills on the bed side table, which looking back, look like hiv pills. And his attitude, he is avoiding me, will not answer my questions. It is as if I have found out his secret. He used to be a drug addict, IDU, how do I know? Because since he had his way with me he posted a video on the internet, telling his life story. Oh yes, he has lost all his hair too. He is bald. It is the sorriest thing I have ever done in my life, because I have a beautiful daughter, and I should have thought of her, before having if off with him. What a unbelieveable fool i am. Thanks for your answers. You obviously know quite a bit about HIV. Are you a HIVer? Or who? Thanks Again.

PS: I am in Ireland,he is in England. Plus he is avoiding me. The privacy laws in england are strict, so I cant get him to do a test unless I test positive. I dont know any of his girlfriends. That is a good idea tho. I'll think about that.

From the sounds of it you've tested negative well past the 6 month mark unless you have some kind of primary or secondary immunodeficiency that test is conclusive. A CD4 count of 500 doesn't mean you have an immunodeficiency, I just want to clarify that.
I see how you could be somewhat concerned about your CD4 being on the very bottom end of the normal range but it is still in the normal range it being 500-1500. Don't read too much into your CD4 count while yes some people with recent infection can uncommonly have CD4 counts in the 200-500 range. There are also people who have HIV who have 1000-1500 CD4 counts. HIV infection can't be the cause of your CD4 count. If you had elevated CD8 cells that would be indicative of infection but it wouldn't necessarily mean HIV. Basically without a positive HIV test very little can be gained from getting a CD4 count test.
I understand some of your concern about him being skinny and having a gut there are numerous reasons for something like this. I know one of the girls I saw her X BF had this same body type really skinny arms but a beer belly given all of the strange circumstances him having HIV did cross my mind(after all HIV is a wasting disease) but it's also possible he just drank alot, has bad genes, had a bad diet and didn't work out.
Where I live also only infectious disease specialist can order HIV RNA PCR's since they're quite costly as it's used as a monitoring tool for HIV infected individuals. It can also be used for diagnosis during the acute stage before the body has mounted a detectable antibody response to HIV. It can also be used as an aid for diagnosis for people with significantly compromised immune system such as CVID, chronic IV drug use, terminal cancer, chemotherapy, or on strong Immunosuppressive drugs for organ transplant such as tacromilus.
If you had one of these conditions I think you'd know about it. Take me for example with my selective IgM deficiency I've had more than my share of health problems already, chronic fatigue at times, skin issues, a strange very painful infection, allergic rhinitis(sinusitis so bad I had to get endoscopic sinus surgery as well as I get allergy shots and it's still not pleasant), recurrent cold sores. I've been on antibiotics for alot longer than most people my age have due to my sinus issues and skin problems and this one strange very painful infection I had. Albeit I was doing fairly well given everything health wise until I had two causal encounters and then I got severely ill with ARS type symptoms a rash, a fever, my skin got inflammed(seborrhea), extreme fatigue, half a dozen or so cold sores, diarrhea, malaise, painful neck lymph nodes, neurological symptoms, bruises on my thighs out of no where, a significant loss of appetite, I also lost weight as well as some strength in the gym despite the fact I forced myself to work out consistently thank god for caffine and energy drinks even while being so sick ( I was practically bed ridden) shoving as much foot down my throat as possible with my decreased appetite.
All of this and I'm still symptomatic months later, a very noticeable loss of appetite(not feeling hungry) if I didn't know how to eat when not hungry I would have lost quite a few pounds, loose stools, fatigue, neurological symptoms to name a few of my more noticeable symptoms.

I know it is difficult, but you need to get tested and put this entire matter behind you.
Most of us on this site have one regret or another.
Try not to beat yourself up too much about this. Testing is the only way you will know YOUR status, no matter what his might be.
