as told by dr. mark The p24 antigen is the core protein of an HIV viral particle which surrounds the viral RNA. This core is in turn surrounded by the outer envelop protein. P24 antigen or protein is produced in abundance and excess when there is lots of HIV replication going on. Thus it is a relatively good marker of acute infection. It appears within a few days in the blood, but goes away within a couple of weeks (after p24 antibodies begin to develop). It is gone for many months to years until AIDS or severe progression of disease develops. Before viral load, we thought we could use it to measure treatment response. It turned out to be a very poor marker for that. It is no longer used in routine clinical practice, but is now manditory for blood banks to screen all donor units with both HIV antibody and p24 antigen tests. Some people use it still for diagnosing acute HIV infection, but viral load testing would be a more sensitive test. MH
