Inconsistent/improper use of condom

Hi and thanks for answering my question. I'm worried about HIV infection from improper use of a condom. When after having sex, how would vaginal fluids not find its way to the penile urethra while one is slipping the condom off? what are the chances of that happening and is there a "proper" way of removing the condom without exposure?
