should i be worried about getting shots with dirty needles? i mean in clinics. i did not see if the nurse got a new one or not . should i?

NO...nor should you worry about tainted cheeseburgers, facial douches, red spotted toilet paper, doorknobs, or canned bloody tuna, breast secretions on your skin, ....and the list goes on*
