What has to happen for an infection to take place? 1) There is no evidence that HIV is transmitted through saliva, and by now the medical community would probably have seen it if it was going to happen. 2) Since saliva doesn't transmit HIV, and since you're asking specifically about an HIV male infecting an uninfected female through oral sex only, that pretty much leaves blood as the vehicle for infection. 3) So the male would have to be bleeding from the mouth, nose or lip, and that blood would have to find its way into an opening in the vaginal tissue. That opening could be microscopic or visible in the form of a sore, lesion, or very small tear. 4) The male's viral load would have to be high enough to cause infection.
