Can there be a tiny puncture at the tip of the condom (used in protected sex) that goes unnoticed?

Sounds like you are engaged in a paranoia-fuelled game of "what if".
However, basically, the answer to your question is NO. From what I understand, a condom is rather like a balloon and if it fails then it either bursts or rips quite dramatically. So you would definately notice a failure. If you did not notice such a failure then your sexual incident was indeed protected and HIV is not a concern for you.
All the best.

No, if the condom broke, it cannot go unnoticed.
