AIDS Service Center NYC
Provide comprehensive case management services for a team (CM/ACM/CFW) caseload of 30-50 people living with HIV and their family members. Responsibilities include but are not limited to: implementing case management service plans, providing escorts for appointments, conducting home and hospital visits, contacting both the provider and the client to ensure that the client received needed services; conducting case conferences, providing short-term crisis intervention and educational counseling, assisting in entitlements advocacy, including budget preparation for income maintenance, DASIS, food stamps, and escorts for fair hearings
