- Conduct comprehensive intake interviews to assist in eligibility determination for the Home Health Program. Develop and monitor service plans for clients. RN License from State of California. Two years clinical experience post RN requried; one year experieince in community nursing preferred. BSN and PHN preferred. $37,500 - $46,900
