Job description: New York Harm Reduction Educators, Inc (NYHRE) is New York’s largest harm reduction/ syringe exchange program. Its mission is HIV prevention and comprehensive services for those at risk for, or infected with HIV. Legally constituted in 1992, we serve over 5,000 people each year in the Bronx and East Harlem using a unique street-side service delivery model. Mobile units and tents are set up where high-risk behaviors take place and services are delivered right on the streets. Services are also offered at two storefront offices, one in the Bronx and one in East Harlem. Position Summary: The ANIMO Case Manager will have day-to-day responsibility for non-clinical program supervision including oversight of program deliverables, coordination with the AIDS Institute and Quality Assurance. The Case Manager will provide case management to high-risk Spanish-speaking substance users and their sexual partners, as per the guidelines for the Psychomedical Intervention Model pioneered in Puerto Rico. S/he will work with the ANIMO Physicians Assistant/ Nurse Practitioner (PA/NP) to deliver the full suite of ANIMO services. The primary recruitment and screening responsibility for the ANIMO Program will lie with the Case Manager. Responsibilities: •Act as project coordinator for Proyecto ANIMO. Provide non-clinical supervision to the PA/NP and any volunteers or other staff assigned to the program. •Responsible for the collection, compilation, submission and basic evaluation of data required for AIDS Institute grant. •Perform non-clinical quality assurance activities to ensure the integrity of the delivery of the Psychomedical Intervention Model. •Participate in and oversee all program evaluation activities in conjunction with the AIDS Institute and the Centers for Disease Control and Prevention. •Work with the ANIMO PA/NP to assess the psychosocial health status and needs of participants in the ANIMO Program. •Provide concrete case management services to all persons enrolled in the ANIMO program, including eligibility for assistance, food/shelter needs, medical, transportation, mental health/ substance use assistance. •Provide and arrange for services needed to remove barriers to improved quality of life and prevention of HIV, hepatitis and STI’s. Promote and facilitate access to all NYHRE services. •Serve as advocate, translator and educator for participants in proper use of community services. •Conduct outreach at street-side sites. Educate and recruit participants into the program. •Establish and maintain active and positive linkages with other community agencies and contribute to the NYHRE linkage agreement file. •Other duties as assigned. Experience: •Bachelors’ degree in social work, counseling, health education, psychology or related field or equivalent experience. •1-3 years case management or client advocacy experience desired. Experience coordinating programs and evaluating performance preferred. •Should be an organized, self-motivated person, committed to working in a multi-cultural, multi-racial, harm reduction environment •Experience working with or desire to work with disenfranchised populations utilizing a harm reduction modality. •Knowledge of the Bronx and East Harlem IDU/ Substance Use communities and service providers preferred. •Experience in the areas of substance use, HIV prevention, STI’s, and reproductive health desired. •Must be willing to work outdoors at our street sites from time to time, hours may vary. •Must be a team player able to work as part of a larger organization with multiple programs. •Bilingual (Spanish) required.
