The Case Manager at Travers House provides on-going case management to the 14 residents of transitional housing. In a 'visiting caseworker' capacity, she/he also works with the 30 tenants in permanent housing to provide crisis intervention, advocacy, referral, and treatment monitoring as needed.
Assists House Manager in marketing the program and recruiting appropriate tenants (permanent) and residents (transitional). Facilitates self-help, support, and recreation groups - planning, convening, assuring (equal) participation - and trips.

Qualifications Either: (a) Bachelor's degree and 1 yr experience in human service delivery, (b) high school diploma and 3 yrs experience, or (c) satisfactory equivalent combination. Must possess strong knowledge of HIV/AIDS, substance abuse, and mental health issues; have good oral and written skills (including public speaking), and be able to work effectively with a poly-diagnosed population. Valid NYS driver's license required. Computer experience required. Some local travel required.
