FOLLOWING THE EMPLOYMENT OPPORTUNIES:

ASSISTANT SUPERVISOR OF CASE MANAGEMENT-FULL TIME
Provide supervision for Case Managers working with people living with
HIV. Maintain a small caseload. MA/MS w/2 years of supervising Case
Managers preferred or BA/BS w/4years exp.

SUPERVISOR-CRIMINAL JUSTICE PROGRAM-FULL TIME
Manage program providing discharge planning for people living with HIV
being released from state correctional institutions and returning to the
Phila. area. Supervise staff and maintain a small caseload. 2 yrs of
criminal justice experience, MA/MS in human services preferred.

PREVENTION SPECIALIST-FULL TIME
Provide prevention education, risk reduction and HIV testing to people
with a history of substance abuse. High school diploma/GED required.
Some college and/or CAC preferred and 2 yrs. experience in behavioral
health/addiction.

CASE MANAGER-FULL TIME
Responsible for conducting intakes, psychosocial assessments, developing
care plans and linking HIV+ individuals with needed services and
resources. Maintain caseload and written and computer database
documentation of activities. BA/BS in Health, Human Services, Sociology
or Social Work. One year of experience in Case Management or Health Care
preferred (field placements may be substituted for paid work
experience).
