|Health Insurance applications for employment

I have been recently hired by a company and after 90 days I will be elligible for group health insurance.I have had creditable insurance coverage continually through COBRA and Inclusive Health which are both HIPAA eligible plans.I have great concerns about disclosing my HIV status when I fill out the application for group health insurance with this employer. I'm not quite sure how to go about this when the time arrives to fill out the health insurance application. How does this work and what should I do ????

Typically, a group health plan offered by an employer does not require an employee to answer a health questionnaire. However, if the health plan has a pre-existing condition exclusion it, you would have to provide a Certificate of Creditable Coverage (from your prior plans). None of this would disclose yourt medical condition.
