| LTD Insurance Info. Request Aug 23, 2005 I've been receiving LTD insurance payments from a former employer's policy since 1997 and my AIDS disease doesn't allow me to work. The original insurer (or servicer) changed to The Hartford. They are requesting a variety of forms for their file, one of which is an "Authorization to Obtain & Release Information." This release authorizes them to obtain information from doctors, insurers, government, businesses, etc., and the information can be released to their insurance companies, The Index System, other doctors, lawyers, etc. I don't feel signing this, especially reading about insurance company tactics to discontinue paying benefits. All of the information I have from my former employer about the policy does not state anything about this. Am I required to sing this? |

Under a disability policy, the insurance company has the right to require proof of disability to continue payments. If you fail to provide this, they could cancel your coverage. | |||||||||
