If the company is offering a group plan, there should be no need to provide medical evidence on one employee. The plan might be community-rated like many HMO's are. However, the cost for insurance will depend on the size of the company, i.e., how many people will be covered under the plan and if you have a plan in place. the plan experience for prior years.
