Is HIV transmitted from a male to female partner during intercourse, only when there are lesions or tears within the woman's vagina? Would this be the only way for the virus to actually enter into the woman's bloodstream, thereby infecting her?

Response from Dr. Luzuriaga

While HIV transmission is probably more efficient when lesions are present, it can also occur when overt lesions are not present.
