AIDS and fertility

The birth mom to my 2 adopted children has AIDS. She has never, and will not take medications nor have a sterilization process. Of the 4 children, 2 are HIV positive. Her health continues to deteriorate, last pregnancy she was down to 70 pounds. While we love these kiddos dearly, I wonder at what point in the illness would a woman not be able to maintain a pregnancy, or is this the last thing to go??

Cross sectional studies have shown that fertilty is decreased in HIV infected women, and the more symptomatic the woman, the greater fertility is decreased. In addition women with advanced disease are at greater risk for preterm delivery and subsequent infant mortality and low infant birthweight in general. However, there does not appear to be a certain threshold of disease below which a woman cannot maintain a pregnancy.
