rising CD4
Apr 6, 2006

i found out i was hiv+ when i was pregnant and immediately started treatment. my cd4 was 288 then and v/l 15000. i became undetectable and i had an hiv- baby. i had a blood test 2 months after birth and after stopping meds and my cd4 was up to 376 and v/l had risen to 22000. could you explain these changes in my cd4 count. do you think it will continue to rise?

Response from Dr. Luzuriaga

With a CD4 of 288, most experts would have recommended treatment to optimize your health and to minimize the risk of transmitting the virus to your baby. You don't mention what happened to your viral load on therapy but it looks as if you derived benefit from the therapy, with a rise in CD4 count. With a baseline CD4 of 288, many experts would have recommended continuation of therapy after the birth of your child. Your CD4 count may remain above your baseline for a while off therapy but it is difficult to predict how long; it is unlikely that it will continue to rise. If you remain of antiretroviral therapy, I would recommend follow-up of CD4 counts and viral load every 3 months. You should strongly consider re-starting therapy if your viral load rises and/or your CD4 count drops below 350 again.
