travelling & medication timing

dear doctor i'm taking meds 2 times per day (combivir+400mgcrixivan+100mgnovir) vl=200 t4=630. i'll have to travel to South America from Europe.i take my meds at 8am & 8pm (when it s 4am &4 pm in Brazil).what would be your advice so that i could take those meds at 8am & pm local time in Brazil.same question for the return to Europe. Thank you very much for your very valuable forum. D.

Dear D.

The safest way to adjust to a new time zone is to move your dosing by one hour a day. So, for an example, you might start 2 days before departure. On day 1 you switch to 9am and 9pm (gmt), then on day 2 you move to 10am and 10pm. On the day you arrive, you will be do to take it at 11/11, which is 6am and 6pm in Brazil. Assuming you don't want to keep waking up at 6am in Brazil (!) you then go to 7 am/7 pm, and so on at a rate of one hour a day. You may want to take a small travel alarm with you and keep it set to home time until you have shifted over. On the way back, repeat the process.

Depending on the regimen, a shift of two hours at a time may be reasonably safe as well.
