I have been on Kaletra/truvada for four years with complete viral suppression.The problem is nausea and diarrhea. Its getting old after four years. My doc wants to switch me over to Intelence from kaletra thinking this will help with the upset etc.If i don't tolerate intellence i can go back on kaletra.I trust him completely but always like to get another view.Does the switch seem reasonable to you? thanks

Sorry to hear about the GI problems, glad to hear that you have managed to stay undetectable.

The choice of intelence indicates that you might have previously had a virus that was drug resistant to an NNRTI (efavirenz, nevirapine or delavirdine). The effectiveness of Intelence would depend on the mutations that your virus had built up to those medications. If that information is known it may be fine. Also if your virus had previously developed resistance mutations to the NRTI class, it might effect the potency of the truvada. Use of intelence with 2 NRTIs after a prior NNRTI failure is not generally recommended (based on a study in which the combo did not work so well due to fairly extensive prior resistance that had not been accounted for). Just be sure that the resistance mutations from your virus would not compromise the new regimen. Your doc should be able to review this with you. Good luck, Joe
