| VL & CD4 COUNT Aug 31, 2008 My VL count keeps on going up and down i.e between 900 and 300, whilst my CD4 count continues to increase every three months.What could b the problem? the VL only reach a level of 80 once and never undetactable, do you think i have a resistance? i take aspen lamzin twice a day and stockrin one a day. |

Based on your question and the fact that your CD4 count "continues to increase every three months", that you have been on your regimen for at least 6 months. If so, your viral load should have reached an undetectable level at this stage. The fact that your CD4 count continues to rise is not surprising since your viral load (even it is not undetectable and hovers around the 300-900 copies/ml range) is likely much less than the viral load you started with prior to treatment. Going back to your HIV regimen (ZDV, 3TC and efavirenz), I am concerned that your viral load should have been undetectable at this point. The fact that it is not undetectable would suggest drug resistance. However, most Genotypic Drug Resistance testing, requires viral load of at least 1000 copies/ml I would suggest going back to your doctor to discuss treatment options. Given the advent of second generation NNRTIs, it is not a good idea to stay on a failing regimen that contains first generation NNRTIs (such as efavirenz) since accumulation of NNRTI resistance mutations may adversely impact on the potency of drugs such as etravirine. |
