Hola, hace 7 meses me detectaron VIH por presentar Tuberculosis Pulmonar, inicialmente mi carga viral fue mayor a 500 000 copias con CD4 menor a 200, al mes de detectarlo y haber iniciado tratamiento con Rifampicina para la Tuberculosis, me iniciaron el tratamiento antiretroviral con Saquinavir-Ritonavor y Combivir,pero mi condicion no mejoraba alparecer no fue la mejor eleccion pues no era recomendable por tomar Rifampicina asi que a los tres meses de llevar ese esquema me cambiaron a Efavirenz y Comvivir, (tambien por presentar Sarcoma de Kaposi en paladar), sin embargo mi condición mejoro mucho,(no me han vuelto hacer conteo de carga viral y CD4) ahora que he terminado el tratamiento con Rifampicina para la Tuberculosis me han cambiado nuevamente a Saquinavir-Ritonavir y Comvivvir,1.¿Que tan recomendable es cambiar nuevamente de esquema? 2- ¿Debo seguir con Efavirenz y Comvivir? 3. ¿Qué es lo mas recomendable? De antemano agradezco su atención y pronta respuesta.

Hola,
Necesitas pruebas de sangre urgentes. ¡Hace 7 meses que te hiciste los últimos CD4 y carga viral! Te recomiendo que no cambies nada hasta que te hagas una carga viral y un conteo de CD4. Si el régimen de efavirenz + Combivir está suprimiendo tu carga viral y aumentando tus CD4, ¿por qué cambiarías nuevamente a tu antiguo régimen que es en realidad inferior al uno actual? Debería yo añadir que con el tiempo deberías pensar en sustituir Combivir por algo que no contiene AZT, un fármaco asociado a efectos secundarios desagradables, tales como la lipoatrofia y la anemia. Piensas en Truvada (tenofovir + emtricitabina) o Epzicom (abacavir + lamivudina), por ejemplo.
