DR. Steve, le escribo nuevamente porque me parece que no llegan mis mensajes.
Hace poco más de un mes me diagnosticaron positivo con coinfección de hepatitis B.
Mi carga viral es de 3500 y mi recuento de CD4+ es de 345.
Tengo en curso ecografía hepática y de vías biliares. Transaminasas al doble del valor normal.
Mi infectólogo ordenó otro recuento de CD4+ para evaluar comienzo de tratamiento. Pero una segunda opinión sugirió no comenzar debido al baja carga viral y recomendó tranquilidad por el tema de HIV hasta carga viral de 100.000 y seguimiento para hepátitis B.
Cual es su opinión?

Querido porteño,
Si el segundo recuento de CD4 corroborara el primero, es decir fuera inferior a 350 células, una tercera opinión (la mía) sugeriría pensar seriamente en comenzar terapia. Las directrices de tratamiento actuales (puestas al día en enero de este año) ahora recomiendan la terapia cuando los CD4 se mantienen constantemente por debajo de 350. En cuanto a empezar tratamiento o no, la carga viral es un criterio de interés secundario: el recuento de CD4 siempre vence la carga viral como un criterio.
