¿Que hacemos el grupo de pacientes detectados en la fase aguda?

Prezado Dr Steve,
Vivo en Brasil y agradezco toda la labor que realiza.
He sido detectado con HIV en octubre de 2007 y el 1 de noviembre de 2008 inicié un tratamiento antirretoriviral.
El equipo de especiliatas me anticipó que- al haber sido detectado en la fase aguda -en un período de 10 meses aproximadamente- dejaría de tomar los antirretrovirales si la carga viral siguen siendo indetectable y los CD4 altos.
Asimismo, me informaron que una vez que deje de tomar los antirretrovirales debo hacer controles periódicos para conocer la evolución de la carga viral y CD4.
He leido que sólo el 1% de los que estamos afectados con HIV hemos sidos detectados en la fase aguda. He leído que hay muy poca investigación sobre este En su opinión
1.-¿Qué sugiere que hagamos los pacientes que fuimos detectados en la fase aguda?
2.- ¿Cuál es el estado de la cuestión en este tema?
3.-¿Qué deben hacer los equipos médicos y los grupos de investigación en situaciones como esta?

Hola Carlos,
¿Has "sido detectado con HIV en octubre de 2007 y el 1 de noviembre de 2008 (¡!) inici(aste) un tratamiento antirretroviral"? Es un tema nuevo en este foro: El tratamiento futurista. Bueno, entiendo. Tuviste la intención de decir "1 de noviembre de 2007".
En serio, como muchos expertos hoy en día, estoy a favor de empezar la terapia durante la fase aguda, o sea el periodo dentro de seis meses después del contagio, y de continuarlo indefinidamente si es bien tolerado. Distinto a tu equipo de especialistas, no recomendaría que dejes de tomar los antirretrovirales si tu carga viral siguiera siendo indetectable y tus células CD4, altas. Si lo haces, lo que pasará es muy previsible: La carga viral aumentará y los CD4 caerán con el tiempo. Es tan sencillo.
