Dr. Steve,
antes que nada muchas gracias por el tiempo que se toma en responder.
Estuve enviando varias veces mis preguntas pero aparentemente estas no están llegando y me encuentro bastante angustiado con mi situación.
En febrero me hice los análisis y resulté positivo en HIV, con transaminasas alteradas (TGO 63 y TGP 84). A fines de febrero recibí mis análisis de HBV y resulté reactivo (HBsAg). En esa fecha mi carga viral de HIV resultó de 3.500 y mi conteo de CD4+ fueron de 345 y porcentuales de 16,6 %.
Mi infectólogo (aparte de derivarme a un hepatólogo) me mencionó sobre la posibilidad de iniciar tratamiento y ordenó otro conteo de CD4+. Estos me los realicé la semana pasada y sus valores son 312 y porcentual de 19,4%.
Puede ser que bajen tan rápido con una carga viral tan baja?
O es más importante el valor porcentual en mi caso?
Mi hepatólogo pidió los demás exámenes de HBV incluída carga viral y posible biopsia. El hepatograma esta vez arrojó una baja en las transaminasas (TGO 53 y TGP 74).
Cuál es su opinión doctor? Por qué bajaron tantos mis CD4+ en un mes con carga viral baja?
Debo comenzar tratamiento?

Hola porteño,
La realidad es que una diferencia tan pequeña en los CD4 o sea 33 células no representa una disminución significativa, porque el número absoluto de CD4 puede variar mucho más de 33 de una medición a la próxima sin indicar nada. Además tu porcentaje de hecho aumentó. Dicho eso, tu recuento de CD4 sin embargo está cayendo repetidamente por debajo de 350 y según las nuevas directrices de tratamiento esto justifica pensar seriamente en comenzar tratamiento.
Mucha suerte con ambos el VIH y la hepatitis B. Un abrazo caluroso de San Francisco soleado.
