CARGA VIRAL CUATRIPLICADA, ESPERO SU OPINION

Estimado doctor le esscribo nuevamente ya que recibi los resultados de mi tercer examen de cd4 y c.v.
Para recordarle mi primer examen en agosto/2006 fue cd4 616 cd4% 7.9, carga viral 60.100 log 4.8, segundo examen enero/07 cd4 495 cd4% 10.1, carga viral 33.400 log 4.5 y mi tercer examen junio/07 cd4 357 cd4% 8.9 carga viral 183.000 log 5.3.
Como vera mi carga viral se me ha disparado.
Mi perfil hepatico es de ALT/GPT 48 PROTEINAS TOTALE 9.36 GLOBULINA 5.26, demas valores valores del perfil hepatico normales al igual que los valores de triglicéridos, hemoglobina , ect., tambien tengo trombocitopenia leve.
Mis preguntas son las siguientes: 1- a la luz de la carga viral tan alta ¿ debo comenzar YA con el tratamiento o esperar un nuevo examen?
2- en caso de comenzar¿ que regimen me aconseja para HIV/HBV CRONICA?, ya que no se si por Uruguay hay pruebas de genotipo.
No lo molesto mas por ahora y dios lo bendiga por su labor, saludos al doctor Bob, aunque escribe en Ingles y no entiendo nada.

Hola nuevamente,
La primera pregunta a la que necesitas abocarte es si la hepatitis B (VHB) crónica requiere tratamiento o no. Tu ALT es normal, pero no sé los otros resultados serológicos (HBeAg), ni la carga viral VHB ni los resultados de una biopsia hepática. Te sugiero que consultes a un hepatólogo para decidirte por tratamiento o no. Esta decisión influirá en la decisión de empezar con el tratamiento anti-VIH o no. Si el tratamiento de la hepatitis B resulta ser apropiado, porque tus CD4 están por debajo de 500, te recomiendo que pienses seriamente en empezar un régimen anti-VIH que incluye tenofovir (Viread) y emtricitabina (Emtriva) (si tu prueba de resistencia no muestra alguna resistencia a estos dos fármacos), porque estos dos drogas son activas contra el virus de la hepatitis B así como el VIH. Si no hay ninguna necesidad de empezar una terapia anti-VHB a este momento, te sugiero que repitas la carga viral y los CD4 en tres meses porque tu porcentaje de CD4 sigue siendo estable y porque sólo una carga viral más elevada que las otras no hace una tendencia.
