Apreciado Dr. Natterstad, Dios lo bendiga por su encomiable labor. Te consulte el 3 y 18 de agosto de 2007 y me habías expresado lo toxico que era mi régimen de Kaletra y Sustiva.
De inmediato, me realizaron un segundo Resistance Report (09/17 07) que dice que no tengo resistencia a NonNucleoside RT Inhibitors y no resistencia a estos tres inhibidores de proteasa : No evidencia de resistencia a: 1.Atazanavir (ATV), 2 Tipranavir+ritonavir(TPv/r),3 darunavir + ritonavir (DRV/r) Todos los demás resultados son Posible resistencia o resistencia.
De todas maneras mi medico me ha dejado en Kaletra, dos al dia (lopinavir 200mg + ritonavir 50mg) (posible Resistance) + Sustiva (Efavirenz 600mg) una tableta al día. (No evidence of resistance) y me ha añadido un tercer medicamento como booster: Epivir (lamivudine) 150mg, una tableta dos veces al día, aunque el reporte dice: Resistance to lamivudine (3tc)/emtricitabine(FTC). Le pregunto si usted fuera mi medico me recomendaría este nuevo esquema? Es bueno para mi?
He tolerado muy bien el Kaletra y el Sustiva mis cd4 están en 323 y mi carga viral indetectable, tengo dudas con Epivir, nunca la he tomado y no se si se asocia con la lipoatrofia.

Hola Ángel,
Te tengo una pregunta de crucial importancia: ¿Cómo es que obtuviste los resultados de una prueba de resistencia cuando tu carga viral es indetectable? Para hacer esta prueba hay que ser una carga viral suficientemente elevada, típicamente de 500 a 1.000 copias. Sin embargo, aunque los resultados fueran confiables, no estaría convencido todavía de la validez de tu régimen. ¿Porqué Kaletra cuando los resultados dicen que hay "resistencia posible" a él y ya estás tomando Sustiva al que tu virus es sencillo? ¿Y porqué ahora Epivir cuando, según tus preguntas previas, ya estás tomando Truvada, una combinación de Viread y Emtriva, el segundo siendo casi idéntico a Epivir? Cómo puedes ver, no puedo entender la lógica aquí. Tal vez puedas aclarar esta confusión en tu próxima comunicación.
